Notification: Shuttle Bus Service to Get a Vaccine at Okinawa Prefecture Wide Area Vaccination Center (No. 2)

At the Okinawa Prefecture Wide Area Vaccination Center, those who have a vaccination ticket can get vaccinated without prior appointment.

As we have done before, we will again provide shuttle bus service from the university to the vaccination center for students and faculty and staff members to reduce the burden and to accommodate those who do not have access to transportation. We would like to take this opportunity to encourage students and faculty and staff members to receive the vaccination if they wish to do so.
For those who wish to get vaccinated at the center, please register your preferred date at the Forms shown in #4 below.

1. DATE: Every Friday in July (Please refer to the schedule on the Registration Forms.)

2. SUBJECTS/VACCINE: Those getting a 1st & 2nd dose or 3rd dose of COVID-19 vaccine　Takeda/Moderna Vaccine

3. DOCUMENTS TO BRING:
　①Vaccination ticket from your municipality
　②Prevaccination Screening Questionnaire
　③Identity Varication Documents (Driver’s license, Health insurance card, or My Number card, etc. ※Student ID card is not accepted.)
　④Proof of the first time (1st & 2nd shots) vaccination (those getting a 3rd shot)
　⑤Prescription record book (those who have it)

5. BUS RIDE LOCATION (DEPARTURE TIME):
　①Garage of the Main Administrative Building (17:10)
　②North Cafeteria (17:20)
　③Entrance to the Faculty of Medicine (17:30)
　＊Bus leaves from the center around 20:00-20:30. Time may vary depending on the number of people getting vaccination.
